Was your sickness caused by an accident at work or an industrial disease? checkbox.
After the last sickness date, the form has some additional information, such as:
In the event of vomiting/diarrhea please tick to confirm that you have been symptom-free for a min of 48 hours checkbox.
I declare that the information I have given on this form is true and I confirm I am now fit to resume work checkbox.
Manager section: Medical advice has been taken by employee checkbox.
Manager section: I confirm employee may resume work checkbox.
