Employees don't see the full electronic return to work form

I do not see the full electronic return to work form.

The return-to-work form will look different due to the date of sickness. The full form will appear after the sickness day passes.

The return-to-work form before the last sickness date only has the following information:

  • Doctor Consulted checkbox.

  • If you did not visit a doctor please state why not text field.

  • Was your sickness caused by an accident at work or an industrial disease? checkbox.


After the last sickness date, the form has some additional information, such as:

  • In the event of vomiting/diarrhea please tick to confirm that you have been symptom-free for a min of 48 hours checkbox.

  • I declare that the information I have given on this form is true and I confirm I am now fit to resume work checkbox.

  • Manager section: Medical advice has been taken by employee checkbox.

  • Manager section: I confirm employee may resume work checkbox.
